You will perform discharge planning, care coordination and provide education and information on post-discharge management for patients and their families.



Your role will include conducting assessment of patients for medical, psychological, social, environmental, vocational and financial needs.



In addition, you will also promote and develop a network of relations between Restructured Hospitals and Intermediate and Long Term Care Facilities
