Social Worker - Executive Solutions

Role:



The post-holder will work closely with a multidisciplinary team to provide a high standard of professional social work services to clients with a wide-range of needs characterized by substance use disorders and physical, mental health and social needs. The post-holder will ensure clients receive services that are recovery focused, strengths based, culturally competent, person-centered and trauma-informed.



The post-holder will participate in delivering agreed projects (and programs) by contributing to the development of the environment of care encompassing clinical service program areas which include:



1 detoxification

2 residential rehabilitation

3 dual diagnosis

4 out-patient

5 day care

6 home treatment

7 out-reach and

8 liaison consultation services



MAJOR DUTIES AND RESPONSIBILITIES:



1. Undertaking and writing up assessments which meet specified standards and timescales for clients with substance use disorders and physical, psychiatric, cognitive and socio-behavioural presentations.

2. Conducting interviews with clients, their families and carers to assess and review their situation.

3. Organizing and managing packages of support, in consultation with multidisciplinary team, to enable clients to lead the fullest lives possible.

4. Liaising with and making referrals to other agencies.

5. Participating in multidisciplinary teams and meetings regarding, for example, child protection.

6. Assess capacity and obtain informed consent and, where clients lack capacity to consent, to have the ability to work within a legal and ethical framework.

7. Promote a 'recovery' based approach to care.

8. Develop and evaluate service provision for clients.

9. Provide consultation to other professions, promoting the importance of self-determination to wellbeing.

10. Participate in applied research, adding to the evidence base of practice in addictions and related areas.

11. Teach, undertake an agreed program of continuing professional development and attend conferences or seminars to increase the personal and collective understanding of mental, emotional and behavioural states or disorders.

12. Maintain appropriately detailed records to monitor the progress of clients’ treatments.

13. Contribute to strategies that align outcomes and goals with the leadership vision for the company.

14. Assist in the recruitment of staff, talent and skills for all clinical services in conjunction with Human Resources.

15. Contribute to a positive work and learning environment by promoting teamwork, co-operation and harmonious relationships amongst colleagues.

16. Assume any other responsibilities and functions as directed by the Consultant and CEO within the scope of the post.
